Hollins High School Athletics

The required athletic paperwork, including the Physical (EL2), EKG, and NFHS Certificates (4), will ONLY be accepted via www.athleticclearance.com. We DO NOT accept paper athletic forms.

All documents MUST be the most current forms from the FHSAA. We CANNOT accept a Health Department form for the physical.

www.athleticclearance.com INSTRUCTIONS:

1.      Go to www.athleticclearance.com and select FL

2.      If you already have an account, sign in using your previously created username and password. Otherwise, create an account. (Please remember your username and password and do not create new accounts each time you sign in)

3.      Register, using a valid email address.

4.      Complete all required fields. The student ID is the full Student ID in FOCUS. This MUST be correctly entered for each student.

5.      Read and electronically sign the EL3 documents. (student and parent)

6.      Upload the correct EL2 forms, EKG form, and NFHS Certificates.

7.      Transfer students MUST submit a GA4 in the appropriate file section.

8.      Be sure to select all sports that your student athlete intends to participate.

9.      All students are required to purchase supplemental insurance. This is done through www.k12studentinsurance.com, and must be purchased each school year prior to participation. Football players MUST purchase football insurance (this also covers them for any additional sports).

PLEASE BE ADVISED, AN ATHLETE MUST COMPLETE THE ATHLETIC CLEARANCE AT LEAST 72 HOURS PRIOR TO THE FIRSTPRACTICE/TRYOUT.
